Day of Surgery
- Please check in at the front desk to verify personal information and your transportation information
- Please bring a current Photo ID or Passport
- Please bring a form of payment for any fees due upon your arrival
- Forms of payment we accept: All Major Credit Cards (we do not accept CareCredit), Debit Cards, Cashiers Check (no personal checks), Cash (must bring exact amount for the two separate fees)
- Please bring any garments that your surgeon advised you to order for your procedure
- Legal Guardian – A legal guardian must be in the center AT ALL TIMES if the patient is under 18 years of age
